1. Notification of Copyright Infringement

- The Work: Identification of the copyrighted work claimed to have been infringed.
- The URL: Exact identification of where the infringing material is located on Heal Wisely.
- Contact Info: Your address, telephone number, and email.
- Good Faith Statement: A statement that you believe the use is unauthorized.
- Accuracy Statement: A statement, under penalty of perjury, that the information is accurate.

3. Counter-Notification
If you believe your material was removed by mistake or misidentification, you may file a Counter-Notification. This must include your signature, identification of the removed material, and a statement under penalty of perjury that you have a good faith belief the material was removed by error.
4. Repeat Infringer Policy
In accordance with the DMCA, Heal Wisely will terminate, in appropriate circumstances, the accounts of users who are deemed to be repeat infringers.
